This memorial is offered to the more than 50 million users of the Internet, to bring home the reality of Sudden Infant Death Syndrome. If you would like to submit a baby's name or add a memorial to a baby already listed, please print or save the following form, fill it out and send it, with your donation, to:

bullet Corporate ... perpetual listing: $500 donation (a group of employees, or an employee-requested corporate donation)

Please make out your check to "SIDS Network". Your donation is tax deductible to the full extent of the law. 100% of it will be used to support the SIDS Network's mission of eliminating SIDS through research, raising community awareness through education, and providing support where it is needed.
